Dental Lifeline Network of Michigan (National Foundation of Dentistry for the Handicapped)
a.k.a. Donated Dental Services (DDS) Program.  Speak with Karen to find a dentist in Michigan at 517-347-0054 or call 888-471-6334.  This is a non-profit network of dentists who help disabled elderly or medically at risk people obtain dental care who have no other way to do so.

LDS DENTAL PARTNERSHIPS


For our LDS members, we suggest that the first place to begin to get dental help is with either of the two following organizations with whom we have partnerships:

Gary Burnstein Community Health Clinic
Gary Burnstein clinic provides free medical care and dental care to low income, uninsured individuals. (at or below 250% of the federal poverty level) You must go through an application process in order to qualify. $20 registration fee ( if they can pay it) and a proof of income. Exam, radiographs, extractions, oral surgery and fillings. There is a $10 charge for dental hygiene services.

Dentures are charged as the lab fee. (Typically $125-$175) Some people may apply for denture assistance. We ask the patient to pay something—if possible. The cost of the denture depends on what the patient needs.

LDS Bishops can access this clinic by sending an email to the PSP Resource Directors with the person's name and contact information. That will provide a quicker time frame for an appointment.

MICHIGAN - STATE DENTAL PROGRAMS

Healthy Michigan Plan (Effective Spring 2014) Covers individuals who are age 19-64, have income at or below 133% poverty level, do not qualify for Medicare or Medicaid, are not pregnant and are residents of Michigan. This plan also covers dental work. There is a cost of $3 at each dental visit. Enrollees must choose a dentist from providers in the network.

Medicaid Michigan also provides dental coverage.

ORTHODONTIC FINANCIAL AID

Smiles Change Lives - ORTHODONTIC FINANCIAL AID Smiles Change Lives (SCL) promotes increased self-esteem and improved oral health for children from low-income families in need of orthodontic treatment. Our goal is to help families, who cannot afford the full cost of braces, secure treatment for their children by connecting them with orthodontic providers willing to donate their services to deserving patients. 1-888-900-3554

The family pays a $30 application fee and then a total of $600 for the orthodontic work for each child. To see if qualify you can look at a table on this web page: http://www.smileschangelives.org/financial/. You first find your "taxable income" on line 43 of your last year's 1040. Then you look at the chart to see if for your family size, you are below the dollar amount listed. If you are at or below that number you qualify and you send in an application. The application is also on their webpage.
